Fiber Impingement in the Wrist (De quervain Tenosynovitis)
This disease develops when the tendons around the big toe become irritated. The swelling of the tendons is called tendonitis. Pain and tenderness develop from the wrist along the first finger. The pain intensifies especially when grasping or making a fist.
Anatomy of the Wrist
There are 2 tendons running from the wrist to the thumb. The tendons are covered with a thin sheath called synovium. This sheath allows the tendons to slide more easily.
Swelling in the tendons can put pressure on the nerves in their neighborhood, causing pain in the wrist and numbness in the fingers..
Causes of fiber entrapment in the wrist
• Overuse
•Hormonal changes during pregnancy
• On the background of rheumatoid arthritis
• It is most common in middle-aged women.
Symptoms of De Quervain’s Disease:
• Pain extending from the wrist to the thumb. The pain may develop gradually or suddenly. Sometimes it radiates to the elbow. The pain intensifies with grasping objects.
• Swelling can be seen around the wrist, especially on the thumb side. Friction sounds or sensation can be felt with thumb movements.
• Pain with swelling makes thumb and wrist movements difficult.
• Numbness in the fingers may occur when the sensory nerves adjacent to the tendons are affected.
Diagnosis of Fiber Impingement in the Wrist
With a special test called Finkelstein, the thumb is closed under the other fingers. The pain is provoked by angling the wrist to the thumb side. The pain can be severe.
Treatment of fiber impingement in the wrist
The aim of treatment is to eliminate the pain caused by swelling and irritation.
Non-surgical treatment
• Rest with thumb-supported wrist splint
• Medication to reduce pain and swelling
• Avoid movements that provoke pain
• Locally administering drug injections
Wrist Fiber Impingement Surgery
Listening may be sufficient if symptoms are mild.
A splint is used to keep it in a neutral position.
Medication is used to treat edema and pain.
In patients who do not improve with medication, the impingement is loosened with closed or open surgery technique.
