Rheumatism may be one of the diseases that cause pain, especially in the shoulder. Some people work with their hands over their heads. These can be athletes playing volleyball, basketball or hanging curtains. But age is also an important factor in shoulder tears. We can see shoulder pain or shoulder problems in one out of two patients over the age of 60-70. Especially when patients lie down at night, they experience problems and pain. It may occur at rest. Over time, the pain causes restriction of movement. First of all, the patient’s history should be listened to well. It should be determined whether it was caused by a fall or a trauma or whether it suddenly appeared when there was no trauma.
The patient is then physically examined. It is checked whether there is a restriction. Then a detailed examination is performed with X-ray and MR examinations. It is revealed whether there is a tear in the muscle. Depending on the size of the tear, if the tear is small, patients can regain their health with physical therapy after shoulder injection with very low dose cortisone. Complaints of 80-90 percent of patients with small tears can disappear.
However, surgical method is used in more serious tears. If the tear is large, repair becomes difficult, then those patients are referred directly to surgery to save time. If the pain has been present for at least 6 months, if the rotator cuff tear is large, if you have experienced a trauma that will cause a serious acute tear, if there is loss of function, if there is weakness, if it prevents you from sleeping at night, surgical treatment is recommended. Arthroscopic surgery is possible in the surgical treatment of shoulder tears.
